The safest trip home from the hospital starts before discharge: get the written instructions and medication list, set up the house for weaker legs, and plan who will be there for the first days and nights.
Before discharge
- Ask for written discharge instructions and read them before you leave.
- Get a complete, current medication list, including anything stopped or changed.
- Write down follow-up appointments and who to call with questions.
- Ask whether any equipment is needed, such as a walker, shower chair or raised toilet seat.
Set up the house
- Clear walkways and remove loose rugs.
- Add night lights between the bed and the bathroom.
- Put everyday items within easy reach.
- If stairs are hard, set up a bed on the main floor for now.
The first week
- Keep medications on schedule. A simple chart on the fridge helps.
- Make easy meals that follow the doctor's instructions.
- Watch for the warning signs listed in the discharge papers, and call the doctor if you see them.
- Do not do it alone. Line up family, friends or a caregiver for the hours you cannot cover, especially nights.
