Hospital to home care, also called transitional care, is extra help in the days and weeks after a hospital or rehab stay, so recovery happens safely at home and an avoidable trip back is less likely. Aging Well Care provides it across Greater Phoenix.
The first days home are when falls, missed medications and confusion about the discharge plan happen most. A caregiver at home in that window gives your loved one steadier footing and gives the family peace of mind.
What hospital to home includes
- Help getting settled on the day they come home
- Reminders to follow the discharge plan and take medications on time
- Help with bathing, dressing and moving safely while strength returns
- Meals that fit the doctor's instructions
- Watching for changes and keeping the family informed
- Fewer hours as recovery goes well
Who it helps
- Anyone coming home after surgery, illness or a fall
- People leaving rehab or a skilled nursing stay
- Families who cannot be there full time during recovery

How it starts
Call (480) 582-9898 or book a free consultation. We come to you, listen and build a plan together, then match a background-checked, drug-tested caregiver your loved one will be glad to see. The plan is guided by a Registered Nurse and adjusted as needs change. No contract required, cancel any time, and no hourly minimum. From the first visit, you are family.
Questions families ask
When should we call about hospital to home care?
Before discharge if you can. We will plan the first day together so care is ready when your loved one gets home.
Is this the same as home health?
No. Home health is skilled medical care ordered by a doctor, such as nursing or therapy visits. We provide non-medical support at home and can work alongside a home health team.
How long does transitional care last?
As long as it helps. Some families need a week or two, others a few months. Our goal is to help your loved one get stronger, so hours step down as recovery goes well. When they graduate from care, we celebrate, and you can call us again any time.
